Hidradenitis suppurativa is rare, long-term skin condition that features small, painful lumps under the skin. They typically develop where the skin rubs together, such as the armpits, the groin, between the buttocks and under the breasts. The lumps may break open and smell or cause tunnels under the skin. Hidradenitis suppurativa tends to start after puberty. It can persist for many years and worsen over time, with serious effects on your daily life and emotional well-being. Early diagnosis and treatment can help manage the symptoms, keep new lumps from forming and prevent complications, such as scarring or depression.

Causes

 

Hidradenitis suppurativa develops when hair follicles become blocked and inflamed. No one knows exactly why this blockage occurs. Factors that may play a role include hormones, metabolic syndrome, genetics, an irregular immune system response, smoking and excess weight.

Hidradentitis suppurativa is not caused by an infection and can't be transmitted sexually. It's not contagious and is not due to poor hygiene.

Symptoms

Hidradenitis suppurativa commonly occurs around hair follicles with many oil and sweat glands, such as in the armpits, groin and anal area. It may also occur where skin rubs together, such as the inner thighs, under the breasts and between the buttocks. Hidradenitis suppurativa can affect one spot or multiple areas of the body.

Signs and symptoms of hidradenitis suppurativa include:

  • Blackheads. Small pitted areas of skin containing blackheads — often appearing in pairs or a "double-barreled" pattern — are a common feature.
  • Red, tender bumps. These bumps often enlarge, break open and drain pus. The drainage may have an odor. Itching and burning may accompany the bumps. They usually appear in areas where skin rubs against skin.
  • Painful, pea-sized lumps. These hard lumps, which develop under the skin, may persist for years, enlarge and become inflamed.
  • Tunnels. Over time, tracts connecting the lumps may form under the skin. These wounds heal very slowly, if at all, and can leak pus.

Hidradenitis suppurativa usually starts between puberty and age 40 with a single, painful bump that persists for weeks or months. For some people, the disease progressively worsens and affects multiple areas of their body. Other people experience only mild symptoms. Excess weight, stress, hormonal changes, heat or humidity can worsen symptoms. In women, the disease severity may lessen after menopause.

Kshar Sutra treatment of Hidradenitis suppurativa

The role of kshar sutra in the management of hidradenitis suppurativa is excellent. So for as 7 cases treated by us by kshar sutra who were suffering from hidradenitis suppurativa of perineal, gluteal & groin region in that 6 patients were male and 1 patient were female. on probing multipe sinuses and fistulus tracts were found. kshar sutra applied in all the sinuses and fistulus tract.Change of kshar sutra have been conducted till all the sinuses and fistulus tract of hidradenitis suppurativa disappear.  Recovery times in thease cases was 4-6 months according to severity of the disease. 3-5 year post operative review did not show any type of complications in any patients.

Anal stenosis, also known as an anal stricture, is the narrowing of the anal canal, located just before the anal sphincter. Anal stricture is a medical problem that is not common; it may be congenital (a baby is born with a medical problem), but usually if it does happen it is after anorectal surgery, which is surgery in the area of the rectum and anus.

This may cause stricture (when a passage in the body becomes too narrow) of the anal canal, which leads to hard or painful bowel movements. When anal stenosis does occur it is usually after a hemorrhoidectomy, which is the surgical removal of hemorrhoids; this is the cause in 90% of cases of anal stenosis.

Mild stenosis can be treated with stool softeners or fiber (which come from plant foods) supplements (in pill or powder form). When the narrowing of the anal canal is not too much, an operation called a sphincterotomy can be done to correct the problem. This is an operation done on the internal anal sphincter muscle, a round muscle inside the anal canal. In cases where the anal canal becomes very narrow, an operation called an anoplasty is done. This is reconstructive surgery (also known as plastic surgery) of the anus.

Management of anal stenosis by kshar sutra

Role of kshar sutra in anal stenosis is excellent. Kshar sutra is applied in anal stenosis after creating a anal fistula at post anal region just like closed posterior sphincterotomy is done for chronic fissure in ano.The kshar sutra is changed at weakly interval till the tract is cut through. Lastly the index finger is passed easyly in anal canal, next proctoscope is passed to its full length. Now the anal canal is widely dilated. Average recovery time is about 7-8 weeks. Total 10 patients of anal stenosis of vayring degree from moderate to severe  was treated by us by kshar sutra during the period from 2009-2015. The regular follow up study was done 1-5 years, during this period not a single patients complaints any tye of anal problem.

Thus it could be calculated the role of kshar sutra in anal stenosis is excellent without any complication as compared to the modern surgical procedures applied in anal stenosis.

Ayurvedic Colitis Treatment

Ulcerative colitis is a chronic disease that results in inflammation of the colon's innermost layer.

Colitis refers to an inflammation of the colon. Colitis may be acute and self-limited or chronic, i.e. persistent, and broadly fits into the category of digestive diseases. The signs and symptoms of colitis are quite variable and dependent on the cause of the given colitis and factors that modify its course and severity. Symptoms of colitis may include: mild to severe abdominal pain and tenderness (depending on the stage of the disease), recurring bloody diarrhea with/without pus in the stools, fecal incontinence, flatulence, fatigue, loss of appetite and unexplained weight loss.

Ulcerative colitis is a disease that causes inflammation and sores (ulcers) in the lining of the large intestine. It usually affects the lower section (sigmoid colon) and the rectum. But it can affect the entire colon. In general, the more of the colon that's affected, the worse the symptoms will be.

The disease can affect people of any age. But most people who have it are diagnosed before the age of 30.

Experts aren't sure what causes it. They think it might be caused by the immune system overreacting to normal bacteria in the digestive tract. Or other kinds of bacteria and viruses may cause it.

You are more likely to get ulcerative colitis if other people in your family have it.

Irritable bowel syndrome (IBS) is a common disorder that affects the large intestine (colon). Irritable bowel syndrome commonly causes cramping, abdominal pain, bloating, gas, diarrhea and constipation. IBS is a chronic condition that you will need to manage long term. 

Even though signs and symptoms are uncomfortable, IBS — unlike ulcerative colitis and Crohn's disease, which are forms of inflammatory bowel disease — doesn't cause changes in bowel tissue or increase your risk of colorectal cancer.

 

Only a small number of people with irritable bowel syndrome have severe signs and symptoms. Some people can control their symptoms by managing diet, lifestyle and stress. Others will need medication and counseling.

The signs and symptoms of irritable bowel syndrome can vary widely from person to person and often resemble those of other diseases. Among the most common are:

  • Abdominal pain or cramping
  • A bloated feeling
  • Gas
  • Diarrhea or constipation — sometimes alternating bouts of constipation and diarrhea
  • Mucus in the stool

It's not known exactly what causes irritable bowel syndrome, but a variety of factors play a role. The walls of the intestines are lined with layers of muscle that contract and relax in a coordinated rhythm as they move food from your stomach through your intestinal tract to your rectum. If you have irritable bowel syndrome, the contractions may be stronger and last longer than normal, causing gas, bloating and diarrhea. Or the opposite may occur, with weak intestinal contractions slowing food passage and leading to hard, dry stools.

 

Abnormalities in your gastrointestinal nervous system also may play a role, causing you to experience greater than normal discomfort when your abdomen stretches from gas or stool. Poorly coordinated signals between the brain and the intestines can make your body overreact to the changes that normally occur in the digestive process. This overreaction can cause pain, diarrhea or constipation.

 

For most people, IBS is a chronic condition, although there will likely be times when the signs and symptoms are worse and times when they improve or even disappear completely.

Anal warts, also known as condyloma, are growths found on the skin around the anus (rectal opening) or in the lower rectum. Anal warts are caused by the human papilloma virus, which is usually transmitted through sexual contact but not necessarily through anal intercourse. There are many types of human papilloma virus; some cause warts on the hands and feet and others cause genital and anal warts. The same type of warts may occur on the penis, scrotum, vagina or labia.

The time from exposure to the virus and growth of the warts is commonly from one to six months, but it can be longer. During that time, the virus remains in the tissues but is inactive. Many patients with anal warts have no symptoms. In many cases, anal warts may remain unnoticed. They often occur without pain or discomfort. Anal warts are found inside and around the area of the anus.

Anal warts start as small bumps that may be no larger than the head of a pin. Initially, they may be too small to be noticed. As they grow, they can develop a cauliflower-appearance when several are clustered together. They may be flesh-colored, yellow, pink, or light brown.

Since the virus that causes anal warts also causes genital warts, warts may occur on other parts of the body at the same time. In women, they may appear on the vulva, vagina, or cervix. In men, genital warts can develop on the penis, scrotum, thighs, or groin. They may also grow on the mouth or throat of an infected person.

Though rare, other symptoms of anal warts can include itching, bleeding, or discharge from the anus. An infected person also may have the sensation of having a lump in the anal area.

Ayurvedic Rectal polyp Treatment

It is mass or swelling arising from mucosal lining of bowel wall. It may be pedunculated or sessile. The polyp consists of rounded head and long pedicle which is attached to the mucasal surface. It my occur any where in the large intestine but most common site of polyp is rectosigmoidal region.

Clinical feature

Rectal polyp is common in children between 3-11 years of age. Bleeding and protrusion of mass during defaecation replaced it self or digitally is the common complaints of patient. Some times there is no protrusion of mass during defaecation only bleeding is the main complaints.
 

Diagnoses

Digital examination reveals pedunculated polyp freely mobile on rectal mucasa. Sometimes the polyp is delivered out side the anal orifice during digital examination.
 

Treatment

Excision of the polyp by snare. The recurrence rate of rectal polyp after excision is 12-17% as reported in the literature.
 

Kshar Sutra Treatment

The polyp is delivered out at anal orifice and Kshar Sutra is ligated. The post operative review of 140 cases of rectal polyp treated by Kshar sutra did not show any recurrence during last 30 years review.

Ayurvedic Pilonidal sinus Treatment

Pilonidal sinus treatment: Conditions: Intermittent boil with discharge of pus, hairs and blood occurs on sacral region in mid spinal line between natal cleft. In acute phase natal abscess is formed which is associated with pain and fever. Kshar Sutra treatment

45 cases of pilonidal sinuses were treated by kshar sutra during last 32 years. Post operative followup study revealed recurrence in 3 cases. The kshar sutra was reapplied and patient were cured. The kshar sutra did not response in two cases, where radiological examination confirmed osteomylitis of sacral bone. Remaining 40 cases were cured and did not show any type of recurrence during followup study period for 3-5 years.

Ayurvedic Fissure treatment

It is most common diasese of Anal Canal which commonly affects the young and adult persons between age group of 21-40 years. Fissure is a linear ulcer in
the mucocutaneous lining of lower anal canal. There is sharp cutting burning or tearing pain in anal canal during defaecation which persists for some hours
after act is over. Children and elderly persons are less commonly affected of the disease.
 

Causes

Constipation is the most common cause of anal fissure. In constipated patient during passage of hard faecal mass the cutaneous part of anal canal is liable to
rupture or teared resulting in acute anal fissure. Due to repeated trauma during passage of hard stool the fissure becomes chronic and healing is rejected.

Clinical feature

The chief symptom of anal fissure is sharp cutting, burning or tearing agonising pain during defaecation which persists for some hours after defaecation. The
pain is so severe that most of the patients are frightened to go to stool and remains constipated for several days. The constipation with hard faecal mass
promotes the repture of anal skin.
 
A streak of blood with faecal column may be seen. Some time mild to moderate bleeding may occur.
 

Swelling

In chronic anal fissure the ruptured skin becomes oedematus and forms a skin tag at the base of fissure most commenly at 6.0 clock.
 

Discharge

Chronic anal fissure becomes infected with pus discharge from anus, so there is a burning and itching at perianal region due to irritating pus discharge.

 

Treatment

Mild aperient is taken at bed time in the night to soften the stool and pass out easily. Hot sitz bath morning & evening after defaecation is very helpful in
relieving the pain which occurs after defaecation.
 

Diet

Fatty, fast and fried food are avoided, saft palatable light and high fibrous diet is recommended. Two wheeler vehicles, scooter, Motor Cycle, Cycle are avoided
because these vehicles aggravate the anal fissure.
 

Operative Treatment

When all the attempts of medical treatment is failed, surgery is indicated in chronic repeated non healing, Septic or infected and fissure.
Generally 3 types of operations are performed in anal fissure.
1. Forcible four finger and dilatation.
2. Closed lateral sphincterotomy.
3. Open posterior sphincterotomy.
 
The operative results of anal fissure are not always satisfactory. Various forms of complications like rectal incontinence, faecal soiling of under clothes and
recorrences are common problems after operations for anal fissure. Majority of the patients are dissatisfied with these types of modern operations.
 

Ayurvedic treatment of Anal fissure

The treatment of Anal fissure consists of hotsitz bath twice daily morning and evening after defaecation. Medicated oil enema is given twice daily before
defaecation and mild laxative like panch skar chooron One T.S.F. at bed time is given to remove the constipation. Inspite of these treatment if the pain is
persistant in chronic, non healing, infected fissure Kshar Sutra ligation is done under local anaethesia. The results of Kshar sutra treatment in anal fissure is
excellent with high success rate of 98.0% . The post operative complications are nil. 500 cases of chronic non healing anal fissure have been treated by Kshar
Sutra during the year 2002 to Jan  2012 at this centre with excellent cure rate.

Kshar sutra treatment for piles

 

After diagnosis and investigations the Kshar Sutra is ligated into the piles mass under local anaesthesia. All the piles masses are ligated at the same session. Patients is admitted in the hospital for one day and next day he is discharge. Post operatively 5-7 days Antibiotics and Analgesics are recommended. Hot sitz bath twice daily and mild aperient like panchsakar churna is given at night to pass out the soft and easy stool in the morning. 3 ml. Jatyadi tail enema is given morning and evening daily before defaecation.
 

Post operative care

 

Patient is asked to keep active himself and avoid long sitting, strenous works and two wheeler vehicle for one
month till the wound is soundly healed up.

Ayurvedic Rectal prolapse Treatment

Rectal prolapse is a circumfrential prolapse of rectal mucosa through anal orifice during dafaecation, which is
replaced it self or digitally. Generally it occurs during child hood and persists through out the life. Both sexes and all
age groups of patients may be affected of the disease.
 

Causes

Faulty habit of dafaecation durng child hood, chronic constipation with long straing at stool, attack of diarrhoea,
dysentry and irregular stool.

Treatment

Operative
1. Anterior resection
2. Rectopexy
 
The operative results of rectal prolapse are not satisfactory. Recurrences, repeated desire to stool and phychological disturbances are common post operative
complaints of the patients.
 

Thiersch’s Suture

A pursestring  future is applied under deep subcutaneous perianal tissues. The dilated anal outlet is narrowed. The Suturing matterials used are stainless steel wire or prolene. The rectal prolase is stopped.
 

Drawbacks of Sutures

Some times the wire is break up and prolene knot is slipped which needs further suturing. I have treated 27 cases of rectal prolapse by using the silk surgical thread during the last 29 years with good results. 90% cases were cured. In 10% cases the suture teared within 6 months. The partial prolapse reoccured but the patients were satisfied as compared to the prolapse before suture.
